Freshman or new student immunization requirements:

• School Physical
• 3 doses of DTP or DTaP
• 1 dose of Tdap; Td or Pediatric DT
• 3 doses of Polio
• 3 doses of Hepatitis B
• 2 doses of MMR (Measles Mumps Rubella)
• 2 doses of Varicella (Chickenpox)
• 1 dose of Meningococcal conjugate (MCV4)

If proof of physical and immunizations are not provided by September 15, 2022, your student will be excluded from school until proper documentation is obtained.

Dental Requirements:

The Illinois Department of Health requires all Illinois students in Kindergarden, Grade 2, Grade 6, and Grade 9 to have an oral health examination.

• This is required for all public, private, and paroachial schools.
• Examinations must be performed by a licensed dentist, and he/she must sign the Proof of School Dental Examination form (found below).
• Each child is required to present proof of examination by a dentist prior to the May 15th, 2022 deadline.
• Each school must give notice of the dental examination requirement to the parent or guardian of the child at least 60 days prior to the deadline of each year.

Freshman Class of 2026 will need to submit proof of a dental exam by May 15th, 2023.

If a child in Grades 2, 6, or 9 fails to present proof by the deadline, the school may hold the child's report card until: The child presents a Proof of School Dental Examination form, OR The child presents a Dental Examination Waiver form, OR The child is enrolled in the free and reduced-price lunch program and is not covered by private or public dental insurance (Medicaid/All Kids). The child is enrolled in the free and reduced-price lunch program and is ineligible for public insurance (Medicaid/All Kids). The child is enrolled in Medicaid/All Kids, but the family is unable to find a dentist or dental clinic in the community who will accept Medicaid/All Kids and is able to see the child. The child does not have any type of dental insurance and there are no low-cost dental clinics in the community that will see the child. The child presents an exemption based on religious grounds (follow Public Health Administrative Rules), OR The child presents proof that a dental examination will take place within 60 days after May 15th, 2022. These children must present proof of a completed dental examination before attending school in the subsequent year.

 

Again, Freshman Class of 2026 will need to submit proof of a dental exam by May 15th, 2023.

Sophomore:

Sophomore athletes must have a sports physical on file with the school by the first day of practice for their respective sport. Students new to the district will need their immunizations up to date.

Junior:

Junior athletes must have a sports physical on file with the school by the first day of practice for their respective sport. Students new to the district will need their immunizations up to date.

Senior:

The Illinois Department of Public Health has mandated that all students entering Grade 12 receive 2 doses of Meningococcal Conjugate Vaccine (MCV4). If the 1st dose is administered on or after the student’s 16th birthday, only one dose is needed. The meningitis vaccine can be obtained at your physician's office, local health department, and pharmacies.

• 2nd dose of Meningococcal conjugate (MCV4)

Senior athletes must have a sports physical on file with the school by the first day of practice for their respective sport. Students new to the district will need their immunizations up to date.

If your student has asthma, an asthma action plan needs to be filled out and signed by a doctor. Your student is allowed to carry an inhaler with them if this form is filled out and an inhaler can be kept in the nurse's office.

If your child has a severe food allergy, a food allergy action plan needs to be filled out and signed by a doctor. Your student should carry an epi-pen with them at all times and an epi-pen should be stored in the nurse's office in case of emergency.

If your child has a severe non-food allergy, an anaphylaxis action plan needs to be filled out and signed by a doctor. Your student should carry an epi-pen with them at all times and an epi-pen should be stored in the nurse's office in case of emergency.

If your child has diabetes, a diabetes medical management plan (DMMP) needs to be filled out and signed by a doctor.

If your child takes any type of medication throughout the day, a school medication request form must be filled out and signed by a doctor. This includes prescription and non-prescription medications, such as acetaminophen (Tylenol) and ibuprofen (Motrin). The nurse cannot give any medication to a student without first having a doctor's permission.
